Document: Guardianship Order. Include the approximate length, recipient, and deadline if you know them. We’ll review your request and may contact you by email about next steps.
What to know
An appointment of a guardian may be limited by subject, time, or later court action. Its scope matters more than its title.
Identify the court or agency, final order, and any amendments or expiry provisions.
Read the exact powers, limits, names, effective dates, and references to attached plans.
Ask whether the recipient needs the full decision or a current certificate of appointment.
Document guide
These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.
Say whether the appointment concerns a child or an adult. Describe the issuing court or authority, country or U.S. state, date, and whether the source is a final order, temporary appointment, certificate, or later amendment. Include the pages that state when the appointment begins or ends. A care arrangement or signed family letter is not necessarily the same document as an appointment by a competent authority.
Look for the clauses that define which personal or property decisions the representative may make. In a child’s appointment, these may concern residence, education, medical decisions, or travel documents; authority for one purpose does not establish every parental power. For an adult, keep the actual personal, property, and representation powers distinct. Translate conditions, required co-signatures, expiration dates, and restrictions as written; do not summarize a limited appointment as unrestricted guardianship.
The Slovak Embassy’s passport guidance describes an application by a legal guardian for a citizen under fifteen. That consular procedure is different from school enrollment or travel with a minor. Ask the receiving office whether it needs the complete court decision, evidence it remains in force, a birth record, or another document, and what translation or authentication form it accepts. Do not use this child-passport example as an adult-guardianship checklist.
The HCCH adult-protection Convention distinguishes guardianship, curatorship, representation, and measures concerning a person or property. An adult appointment may therefore involve different powers and records from a child’s custody order. Identify whether the source authorizes personal decisions, property administration, or a specific act, and include later restrictions or amendments. The Convention concerns international arrangements between its Contracting States; its existence does not establish that it applies to a particular U.S.–Slovak case. Check its status table and ask the Slovak recipient or qualified counsel which recognition procedure and translation package apply.
Give the issuing jurisdiction, child or adult appointment, record title, page and attachment count, recipient, required format, and deadline. Mention a separate custody order, power of attorney, or later appointment if relevant. Keep identities, health details, case numbers, addresses, and private family facts out of the public quote form; arrange a suitable file-review method after the request is understood.
