What to know
The declarations page, coverage wording, exclusions, conditions, and endorsements can all affect what the English source says.
Include declarations page, schedule, endorsements, and current policy wording.
Limits, deductibles, waiting periods, and exclusions should not be summarized as unconditional coverage.
Ask whether the recipient needs the full policy or a certificate of insurance.
Document guide
These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.
State the insurance type, issuing country and carrier, policy edition or renewal date, number of pages, and why a Slovak reader needs it. A certificate of insurance, declarations page, full policy, claim letter, and endorsement are different records. The California Department of Insurance defines a declarations page, endorsement, and exclusion in its auto-insurance glossary; other products and jurisdictions can be structured differently.
List schedules, definitions, riders, endorsements, and notices effective during the period at issue. The California regulator explains that an endorsement changes policy terms by adding or subtracting coverage. Translate amounts, limits, deductibles, waiting periods, and exceptions as stated; do not summarize a conditional benefit as unconditional. Ask the recipient whether it requires the entire current set or specified sections with their definitions.
The translated text communicates the source; it cannot confirm that a person or loss is covered, decide a claim, or replace the insurer’s interpretation or legal review. If the policy was renewed or amended, identify which edition applied to the event. Ask the insurer, broker, adviser, or receiving office what evidence it actually needs before paying to translate a long policy.
Give product type, policy year, sections and page count, endorsements, source format, recipient, and deadline. Do not put a policy number, address, insured names, claim narrative, or medical facts into the public quote form. A suitable file-review method can be discussed after the initial description.
