Separate the blank form from the completed claim

An insurer’s instructions, questions, declaration, and evidence checklist may all need to be understood before a response is prepared.

01

Source record

Include instructions, definitions, signature consent, and attachment checklist.

02

Reader and purpose

Question wording and answer fields should remain clearly separated.

03

Quote details

Ask whether the insurer accepts translated answers or needs its original form completed.

What to check before ordering

These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.

Identify what needs translation

Tell us whether the source is a blank claim form, completed statement, proof-of-loss form, insurer correspondence, adjuster report, or supporting evidence. Include the insurance type, issuing country, page count, and intended Slovak reader. The California Department of Insurance describes a proof of loss as a formal statement a policy owner may be asked to complete after a property loss; other claim types and insurers use different forms.

Keep questions separate from answers

Field labels, instructions, definitions, declarations, authorization or consent wording, signatures, and attachment checklists should remain clearly distinguished from a claimant’s entries. Dates, amounts, units, and claim categories must match the source. If a form refers to policy terms or an insurer letter, ask whether those documents need translation too. A translated blank form is not evidence that any claim was filed.

Follow the insurer’s live filing process

The insurer controls what language and format it accepts, the deadline, and where the completed form must go. Translation does not submit a claim, fill in facts, or decide coverage. Ask the insurer or claims adviser whether it wants a Slovak reading copy, a translation of completed English answers, or another supporting record. If the deadline is near, contact the insurer directly rather than waiting on a quote.

Protect the claimant’s data

State the form type, pages, attachments, recipient, language requirement, and deadline without sharing policy numbers, medical history, bank details, or a full loss narrative in the public form. We can arrange appropriate review of the actual file after the scope is established.

How to request a quote

Tell us what the document is, who needs the Slovak translation, and when you need it. The first form does not require a file.

01

Describe the document

Choose the type and tell us where the translation will be used.

02

Add your requirements

Include the approximate length, deadline, and any instructions from the recipient.

03

Review and next steps

We review the request and may contact you by email if more detail is needed.

Common questions

Answers to common questions before you request a quote.

Can I submit a Slovak translation instead of the insurer’s form?

Ask the insurer. It sets the accepted form and submission channel for the claim.

Does translating a proof-of-loss form file a claim?

No. The claimant must follow the insurer’s filing instructions and deadlines.

Have an English document to translate?

Start with a few details. No file upload is needed for the first step.
