Document: Sworn Statement. Include the approximate length, recipient, and deadline if you know them. We’ll review your request and may contact you by email about next steps.
What to know
A sworn statement is the speaker’s assertion, not a court finding or a substitute for an authority record.
Include the declaration, oath or affirmation clause, signature, and witnessing text.
Qualifiers such as “to the best of my knowledge” affect the strength of the statement.
Ask whether a specific statutory declaration form is required for the destination procedure.
Document guide
These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.
Record the source title, declarant, date, place, exact oath or affirmation wording, witness or officer, signature block, and any seal. A statement made before a notary differs from an unsworn personal letter. Keep qualifications such as “to the best of my knowledge” and “I believe” in the translation because they affect what the person actually asserted.
A receiving authority may ask for its own declaration template, a record issued by a public office, or an affidavit in a particular legal form. For Slovak residence proceedings, the Foreign Ministry lists sworn declarations for some accommodation situations and sets signature and translation requirements for required documents. That does not make every sworn statement acceptable for every purpose. Get the recipient’s current document instructions before ordering.
If the statement identifies attached records, count them and ask whether they must be translated. The person’s factual assertion, the officer’s signature certification, an apostille, and the Slovak translation are different pieces of the package. Translation should not imply that the officer verified the truth of the contents or that an apostille validates the claim.
Describe the issuing jurisdiction, purpose, page and exhibit count, recipient, required formalities, and deadline. Do not paste names, addresses, family circumstances, or the contents of the statement into the initial quote form. We can discuss a protected review route after understanding the scope.

Frequently asked
Answers to common questions before you request a quote.
The title and legal form differ by jurisdiction. Ask the receiving body what it accepts, and preserve the source form in translation.
No. It conveys the declarant’s words. The recipient decides what evidentiary weight and formalities apply.
