What to know

Identify the trade, awarding body, training dates, and attached modules.
Hours, competencies, and assessment grades may require a consistent term list.
Ask whether the recipient needs the certificate, curriculum, work log, or all of them.
Document guide
These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.
Give the issuing country, trade, training provider or registration agency, award date, recipient, and purpose. A U.S. Registered Apprenticeship can lead to a Certificate of Completion issued by the Office of Apprenticeship or a State Apprenticeship Agency, according to Apprenticeship.gov; other programs use different awards. Check whether your document is that certificate, a school course certificate, a transcript, or an employer letter before choosing the Slovak title.
Include certificate front and back, training period, classroom modules, supervised work hours, assessments, occupation title, issuing signatures, and any linked transcript or work log. For a completed U.S. Registered Apprenticeship, Apprenticeship.gov’s Verify My Apprenticeship tool can provide a downloadable transcript; a replacement Certificate of Completion is requested through the Office of Apprenticeship or relevant State Apprenticeship Agency. Ask the Slovak recipient which record it needs before ordering either. Do not turn a participation record into a completion credential.
If the apprenticeship will support entry to a regulated profession in Slovakia, search the Ministry of Education’s National Regulated Professions Database for the exact occupation and its responsible authority. If it is not listed, ask the employer or school what training evidence it wants; do not assume the professional qualification recognition route applies. A translated occupational title does not establish that the training meets a Slovak qualification or licensing requirement. Ask the actual recipient what evidence and translation form it requires. Preserve original grades, hours, and credential numbers rather than converting them informally.
State trade, issuer country, page and attachment counts, recipient, requested format, and deadline. Tell us if a curriculum or logbook also needs translation. Do not enter the apprentice’s full name, registration number, birth date, employer details, or grades in the public quote form; share the document later through an appropriate authorized route.
