Document: Continuing Education Record. Include the approximate length, recipient, and deadline if you know them. We’ll review your request and may contact you by email about next steps.
What to know
A course-attendance certificate, assessed credential, self-kept log, and credit transcript do not carry the same evidence.
Include provider names, activity dates, credit categories, and the complete record period.
Credit units may not map across systems; retain the issuing body’s terminology.
Ask whether certificates for individual courses are needed with the summary.
Document guide
These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.
Give the professional field, issuing bodies, date range, intended Slovak employer or regulator, and reason the record is requested. Ask whether the recipient wants a summary transcript, individual course certificates, assessed outcomes, or all supporting records. Europass describes learning credentials ranging from participation certificates to diplomas; the exact source wording matters more than a generic translation of “credit.”
Include provider names, activity titles, dates, contact or study hours, credit category, delivery mode, assessment status, expiration terms, and signatures when present. A self-maintained log should remain identifiable as such, and a certificate of attendance should not become proof of an exam pass. Preserve the issuer’s credit unit and number rather than converting it into a different system without the receiving body’s instructions.
The Slovak Ministry of Education explains that a lifelong-learning certificate for a regulated profession is assessed by the competent professional body or the body issuing a similar Slovak certificate; it also distinguishes some education questions from specialty training. The rule for a particular profession and activity must be checked with that body. Translation communicates the record but cannot award or validate professional credits.
State profession, record period, number of entries and certificates, tables, issuer countries, recipient, output format, and deadline. Note whether attachments contain individual learning outcomes or only attendance. Do not enter license numbers, learner IDs, private contact details, or full credential records into the public quote form.
