Carry forward the care and follow-up plan accurately

A discharge summary may be the main written bridge between a hospital stay and the next provider.

01

Source record

Include medication list, procedure notes, follow-up instructions, and attachments.

02

Reader and purpose

Dates, doses, allergies, and conditional instructions must remain exact.

03

Quote details

Ask whether the receiving clinician needs the full record or this summary alone.

What to check before ordering

These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.

Identify the hospital episode

Give the hospital and specialty, admission and discharge dates, document date, pages, intended Slovak recipient, and whether the source is a discharge summary, patient instruction sheet, medication reconciliation list, or another record. AHRQ distinguishes the clinician-facing discharge summary from a patient-facing plan. Ask the receiving team which documents it needs rather than assuming a one-page summary covers the whole stay.

Keep medication changes and pending items visible

Record diagnoses, procedures, treatment course, condition at discharge, medication names and doses, allergies, follow-up appointments, and any test pending at discharge exactly as written. AHRQ’s transition-of-care work highlights medication and follow-up information as important communication. Do not silently resolve conflicting medication lists or infer that a pending result is normal; bring discrepancies to the medical team.

Treat the translation as part of a handover

If the patient is moving to another provider, ask that provider whether it needs the English original, lab or imaging attachments, procedure notes, and direct clinician-to-clinician communication. Translation can make the written summary readable, but it does not replace medical handover or professional review. A clinician should verify treatment instructions and any medication equivalence before they are used.

Scope privately and flag real urgency

Give specialty, approximate pages, tables, recipient type, required output, and deadline without posting diagnoses, names, prescription details, or identifiers into the public quote form. Arrange appropriate file review after scoping. For urgent clinical decisions, contact the medical team directly instead of relying on a website response time.

Common questions

Answers to common questions before you request a quote.

Is the patient instruction sheet the same as a discharge summary?

Not always. Ask the receiving provider which document and supporting records it needs.

Can translation replace a clinical handover?

No. The treating and receiving teams decide how to transfer and verify care information.
