Document: Laboratory Results. Include the approximate length, recipient, and deadline if you know them. We’ll review your request and may contact you by email about next steps.
What to know
An isolated number can be misleading if the test method, specimen date, flag, or reference interval is missing.
Include every page, specimen date, reference range, and test-method note.
Units and comparison symbols must be preserved; do not silently convert or interpret values.
Ask the receiving clinician whether prior results or explanatory pages are also needed.
Document guide
These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.
Give the issuing laboratory, report date, specimen collection date, test panel type, pages, tables, and intended Slovak reader. Include the complete result page, continuation pages, legends, method notes, and any corrected or amended report. Ask the receiving clinician whether prior results or imaging reports are needed to understand the trend. A portal screenshot may omit headings or footnotes from the formal laboratory report.
MedlinePlus explains that laboratories can use different methods, reference ranges, and measurement units. Keep every result with its printed unit, laboratory-specific interval, comparison sign, abnormal flag, and specimen information. Preserve “negative,” “positive,” and “inconclusive” as the source uses them. Do not convert units or replace the printed range with one found online unless a qualified clinical team separately requests and verifies that work.
A result outside a listed interval does not by itself diagnose a condition, and a value inside it does not by itself rule one out. MedlinePlus recommends discussing results with a provider in the context of history and other tests. Translation conveys the lab’s wording and figures; a clinician decides their meaning and whether a repeat or follow-up is needed.
State panel type, approximate pages and table density, source format, recipient, and deadline without posting test values, names, dates of birth, or laboratory identifiers publicly. Arrange appropriate file review after scoping. If an urgent result is involved, contact the ordering clinician or laboratory directly.
