Document: Medical Report. Include the approximate length, recipient, and deadline if you know them. We’ll review your request and may contact you by email about next steps.
What to know
A consultation note, radiology report, pathology report, and medical certificate answer different questions for a Slovak reader.
Include all pages, test attachments, clinician signature, and report date.
Diagnoses, uncertainty, medication names, and units require precise context.
Ask whether the recipient needs the full report or a clinician-issued summary.
Document guide
These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.
State the issuing clinic or hospital, specialty, report type, date, source language, approximate pages, and whether the recipient is a clinician, insurer, school, or administrative office. A clinician’s narrative, test result, imaging interpretation, and fitness certificate should not be labeled interchangeably. Ask the recipient whether it needs the signed full report, a summary from the treating clinician, or specified attachments.
Some reports, such as radiology reports, separate examination details, findings, and the clinician’s impression. RadiologyInfo explains that an impression can include possible causes and recommendations; it is not a substitute for discussion with the ordering doctor. Keep qualifiers such as “possible,” “rule out,” and “cannot exclude,” along with dates, units, abbreviations, and any referenced prior study. Do not turn a tentative statement into a diagnosis.
A report may refer to images, lab values, specimen descriptions, medication lists, or earlier records that are not in the file. Ask the receiving clinician what supporting material is needed for continuity of care and whether an English original should travel with the Slovak text. Translation communicates the recorded wording; it does not interpret symptoms, recommend treatment, or certify a patient’s condition.
Give report type, specialty, pages, tables or handwritten notes, intended use, requested format, and deadline. Do not enter names, diagnoses, test values, insurance identifiers, or a scan into the public form. Arrange a suitable review channel after the scope is clear. If care is urgent, contact the treating or receiving medical team directly.
How it works
Tell us what the document is, who needs the Slovak translation, and when you need it. The first form does not require a file.
Choose the type and tell us where the translation will be used.
Include the approximate length, deadline, and any instructions from the recipient.
We review the request and may contact you by email if more detail is needed.
Frequently asked
Answers to common questions before you request a quote.
No. The treating or receiving clinician should interpret it alongside the medical history and source report.
Images themselves are different from their written report. Ask the receiving clinician what files and descriptions it needs.
