Translate the approved form within the care team’s consent process

A readable form supports a discussion; the clinician and institution remain responsible for the actual consent process.

01

Source record

Include information sheet, signature form, revisions, and linked patient material.

02

Reader and purpose

Risk qualifiers and opt-in or opt-out choices need exact handling.

03

Quote details

Ask the healthcare provider which approved language version and consent process it requires.

What to check before ordering

These notes help you describe your request. The receiving organization’s current instructions should guide the final requirements.

Identify the type of consent and approved version

Say whether the document concerns a treatment, diagnostic procedure, surgery, or participation in research. AHRQ’s guidance on medical-treatment consent explicitly excludes research consent, which follows a different institutional process. Ask the provider for the current approved form, version date, information sheets, and any separate signature or witness page. The recipient should confirm whether it accepts a translated form and who can approve its use.

Keep choices and risk language exact

Provide the full text covering purpose, expected benefit, material risks, alternatives, the option to decline, questions, and withdrawal language if present. Preserve probability qualifiers, negative statements, checkboxes, initials, and the distinction between a required step and an optional choice. Flag unclear or conflicting source wording to the care team rather than silently changing clinical meaning.

Plan for discussion and language support

AHRQ describes informed consent as communication, not merely a signature. The provider should decide how the patient can ask questions and demonstrate understanding, including any interpreter or language-assistance arrangement needed. Translation cannot decide capacity, establish consent, or substitute for the clinician’s explanation. The institution should check the final Slovak form against its approved version before anyone uses it.

Request a quote without patient details

State form type, specialty, version, page count, whether tables or checkboxes are present, intended provider, review owner, and deadline. The public form should not contain a patient name, diagnosis, procedure date, medical record number, or signed form. Arrange an appropriate way to review sensitive source files after the project scope is agreed.

Common questions

Answers to common questions before you request a quote.

Can a patient sign the translated form immediately?

Ask the healthcare institution to approve the exact form and run its consent discussion and language-assistance process first.

Is a research-participation form the same as a treatment form?

No. Identify research consent separately and follow the sponsor or institution’s approved process.
